IMPORTANT: Inland Marine (equipment & contents) coverage is not currently available for purchase through our online process effective 7/1/2023. Instructions

All insured teams in a League, Conference or Association, etc. must be covered under the Combined Plan.
The premium developed by your application will be the minimum premium for coverage. Premiums are 100% fully earned and are non-refundable once the coverage begins.
Full payment must be submitted with the completed application before your Liability Insurance Certificates and Accident Insurance Plan Description will be issued. No deposits will be accepted.
In order to avoid delay in the processing of the applications, please make sure the information you submit is complete and accurate.

Description of Benefits

Who is Protected:

Coverage is provided for the insured League or Conference, its teams, sponsors, officers, directors, managers, coaches, umpires, referees and other managing personnel while acting on behalf of the insured League or Conference or one or more of its teams; the additional interest of the owners of the playing facilities (City, Municipality, School District, etc.)

Major Coverages Afforded by this Policy

Maintenance or use of ball parks and fields, stands and playing areas, including: all activities necessary or incidental to the conduct of practice, exhibition, post season and scheduled games.
Consumption or use of food products
Cost of investigation and defense against claims, even if they are groundless
Non-Owned Hired Auto Liability
Personal & Advertising liability
Participant legal liability
Professional Liability

Excess Participant Accident Medical Expense Benefit

The Plan pays for covered medical expenses starting within 90 days from the date of the accident causing the injury. We will pay all covered medical expenses incurred within 3 years from such date, less the deductible (if any), not to exceed the maximum benefit amounts.

Covered Expenses for this Benefit means the Medically Necessary and Reasonable Charges for services, supplies, and treatment provided or prescribed by a Physician for which an Insured Person is required to pay, except as may be limited in the Schedule of Benefits and subject to all applicable conditions, exclusions and limitations.

Excess Provision

The Accident Medical coverage is payable in excess of all other valid and collectable group insurance (Examples: Major Medical Plans, Automobile Medical Policies and HMO Plans).

Accidental Death and Specific Loss Benefit

If a covered injury causes the death or loss of sight or limbs of the insured person, a benefit will be paid upon receipt of due proof that:

The insured has sustained a loss of the type listed below within 180 days after suffering the injury and
Such loss resulted directly and independently of all other causes from such injury.

Loss of a hand or foot is actual severance through or above the wrist or ankle joint. Loss of an eye is the complete and irrevocable loss of sight of the eye. The total benefits payable under this coverage to one insured who suffers more than one such loss from one accident shall not exceed $35,000.

If more than one loss occurs from one accident only one amount, the larger, will be paid.

Aggregate Limit of Liability

The Aggregate Limit shown in the Schedule of Benefits is the maximum amount payable by Us for all Claims incurred for all Insureds under the Policy which are caused by any one Incident that occurs when the Policy is in force. If this limit is not sufficient to pay the total of all such Claims, then the Benefit payable to any one Insured will be determined in proportion to our total aggregate limit of liability.

Who is Insured

All participants involved in tryouts as well as all participants whose names are registered as eligible team members (ages 16 & under for tackle football and 18 & under for flag and touch football) with the insured league or conference. In addition to the registered participants, the plan will cover cheerleaders, coaches, trainers, managers, volunteers, umpires and referees while acting in their official capacities.

When are They Covered?

The Plan provides coverage while:

Participating in scheduled games and practice sessions (must have adult supervision)
Participating in occasional fund-raising events
Traveling in a group directly to or from a scheduled game or practice session that is directly supervised by the Plan Sponsor or one of its member organizations.

Terms of Coverage Afforded by Combined Plan

Coverage will become effective the day following the date the application and payment are mailed to K&K Insurance (as shown on the postmark), whichever is later.

Accident Insurance policies are provided by Nationwide Life Insurance Company. Liability Insurance policies are provided by Nationwide Mutual Insurance Company.
